How should I prepare for buttock augmentation?

When preparing for buttock augmentation and/or buttock lift, you may be asked to:

  • Have a blood test
  • Take certain medications or adjust your current medications
  • Stop smoking
  • Avoid taking aspirin and certain anti-inflammatory medications, as they can increase bleeding

Buttock augmentation can be performed at an accredited surgical facility, an outpatient surgical center, or a hospital.

If your procedure is performed as an outpatient, make sure someone drives you to and from surgery and stays with you for at least the first night after surgery.

What is the treatment process?

Buttock Augmentation is an outpatient procedure performed in a hospital or accredited surgical facility under general anesthesia.

After the one to three-hour surgery, you should be able to go home the same day.

Beforehand, your surgeon will discuss the implant size and shape with you, as well as the type of incision and location of the implants.

In most cases, a two- to three-inch incision is made near the tailbone through which the implants are inserted and placed, usually below each buttock muscle.

Bandages will be applied, and you will likely need to wear a compression garment for a short period of time.

Your surgeon will provide you with these and other details, as well as pre-and post-operative instructions, at your initial consultation.

How long will it take for me to heal?

We recommend that you go home and relax for about 10-12 days before returning to work and normal activities.

You should not exercise or do any strenuous exercise for about four weeks or until your surgeon suggests it is safe to continue.

The bandages will be removed about two to three days after the procedure. After that, you should be able to bathe and move around relatively normally.

Swelling, bruising, and tenderness in the buttock area are normal and should subside over time.

Buttock contouring – or butt lift for short – is a relatively new procedure in the world of cosmetic surgery, but recently published statistics show that its popularity is on the rise.

This guide describes the various surgical procedures available to improve the volume and shape of the buttocks.

The anatomy of the buttocks

Men and women alike desire attractive and shapely buttocks.

And although the buttocks of men and women vary somewhat in shape and appearance, all buttock shapes share four main aspects that determine their appearance:

  • Bone structure
  • Muscles
  • Subcutaneous fat
  • Skin

Bone structure

Your pelvic bone, which forms the shape of your hips and the support structure for your buttocks, is also the one part of your buttock region that you cannot change.

Women tend to have a wider pelvis to accommodate childbirth, which also causes their thigh bones to be set wider.

This means that the buttocks and hips are shaped wider, features that have long been associated with femininity.

Muscles

Most of the shape of your buttocks is made up of muscle, along with some fat.

There are a number of muscles in the buttock region, many of which help to keep your body upright, but the three main ones that contribute to the shape of the buttocks are:

Gluteus Maximus

The largest and probably strongest muscle in your body, the gluteus maximus, makes up most of the muscle mass in your buttocks. Gluteus medius (buttock muscle)

This muscle is much smaller than the gluteus maximus and sits lower behind it. The gluteus medius is sometimes called the upper gluteal muscle. Gluteus Minimus

The smallest of the three gluteal muscles, the gluteus minimus, is the deepest layer of muscle located below the gluteus medius.

Subcutaneous fat

Subcutaneous fat, or fat that is literally “under the skin,” covers almost your entire body.

Most people think that fat is undesirable or unhealthy, but the truth is that some fat is good and even necessary in some places.

This much-needed layer of fat, also called “adipose tissue” is found in your buttocks as a layer that sits above your gluteal muscles.

Your buttocks need a healthy layer of subcutaneous fat to perform everyday movements, such as simply sitting down. Generally, this layer of fat is thicker in women.

You have female hormones to thank for this, which promote the storage of fat in the buttocks and thigh area.

How this fat is distributed in your buttock area is one of the most important factors in the overall shape and appearance of your buttocks.

This distribution is mainly determined by genetics, hormones, and diet, and exercise.

Skin

The final layer of tissue that makes up your buttocks is your skin.

Similar to other parts of your body, the skin of your buttocks lies over the subcutaneous tissue, holds everything in place, and is subject to the same signs of aging.

Like the skin on the rest of your body, the skin on your buttocks can have spots, cellulite, and stretch marks.

As you age, the skin all over your body, including your buttocks, loses elasticity and thickness. For your buttock muscles, this means that they sag.

The “perfect” butt

Everyone’s butt is different, so achieving the “perfect” butt can look different for everyone.

According to a 2009 study in the Seminars in Plastic Surgery, aptly named Ethnic Considerations in Buttock Esthetics, your definition may even be influenced by ethnicity.

In general, the ideal butt shape is the result of a combination of a few key characteristics.

Back curve

For many women, the curve of their back is a defining feature of their buttocks or at least the top of their buttocks.

Some people tend to have a more natural curve in their lower back, and this helps their buttocks to show a perkier contour.

Why is this attractive? Scientists believe it may have to do with reproductive health: This tilt reduces the risk of spinal injuries during pregnancy.

So if you’re endowed with a curvy lower back, you’re already well on your way to the ideal butt shape and “butt cleavage.” Hips and waist width

This is another visible sign of fertility, according to scientists. Wider hips not only make it easier to deliver babies but, combined with a narrower waist, are a sure sign that you’re not pregnant right now. With a wider waist, you may also store more fat in your butt.

Buttock crease

Another important factor in your ideal butt shape is the definition of your butt crease. This fold of skin marks the bottom of your butt where your thighs begin.

The ideal buttock has a well-defined gluteal crease that clearly separates the thighs from the buttocks.

Thigh

You don’t want to draw attention away from the buttock area, so your thighs should be narrower.

4 Common Butt Shapes

Many women think that the perfect butt is all about size.

In reality, it’s all about the fatty tissue that sits on and around the buttocks.

Muscle is important to lay the foundation and can be worked out for more definition, but it’s the fat that gives you the softer contours of the attractive female butt.

No two butts are exactly alike in shape, but most fit into four general categories.

The V-shape, most often considered the least attractive, occurs when there is more fat in the waist and upper buttock area.

The buttocks taper toward the thighs, so there is no line between the buttocks and the legs. Square-shaped buttocks tend to look flat from the side.

A common complaint is that they look almost masculine.

This is usually the most commonly complained about shape and can be caused by a little too much fat in the “love handle” area around the midsection and inadequate projection.

A round butt is usually considered attractive. This shape has a lot of fat volume in the cheeks and sides.

When there is a significant amount of volume, it is also referred to as a “cracked butt.” The heart-shaped or A-shaped butt is typically considered the “ideal” butt shape.

Imagine an upside-down heart with a smaller waist and larger hips. Most of the fat is located near the bottom of the buttocks, with good projection.

Factors that influence the shape of the buttocks

It can be extremely difficult to maintain the elusive and ideal buttock shape, especially with so many factors negatively affecting it.

As you age, the time has an extremely negative impact on the shape of your buttocks. The genetics you were born with can also play a major role in how your butt changes.

Finally, diet and exercise can have a remarkable effect on the shape of your buttocks.

Aging

Age probably has the biggest impact on the appearance of your buttocks. By the time you reach your 20s, the shape of your buttocks is usually fully formed, and you have a lot of good, healthy fat stored there.

Your buttocks can be firm and high even if you are not physically active and lead a sedentary lifestyle.

Unfortunately, for most women, it goes downhill once they hit their 30s – or even before if you get pregnant.

Muscle mass and tone, skin cell turnover (i.e., the frequency with which the body replaces skin cells), and skin elasticity all decrease during this time. These decreases contribute to sagging and wrinkling.

If you’re in your early 40s, you’re likely to start sagging a bit more, accumulating fat around your hips and love handles and getting a butt that isn’t as high and “perky” as it used to be.

This worsens in your 50s and during menopause.

The drop in estrogen that comes with menopause leads to all sorts of changes in your body, including the storage of more fat around your middle and hips and a loss of fat in your buttocks.

Your butt starts to look flatter and continues to drop.

Once you reach the age of 60, the sagging continues. However, it is important to maintain strong buttock muscles even at this stage of life.

No matter how old you are, your back and hips need strong and stable support to keep you from falling.

Genetics

The way your buttocks are naturally shaped is largely controlled by your genes.

Your genetic makeup determines how the muscle groups of your buttocks are placed and contoured, as well as the width and overall structure of the pelvic bones that support them.

Your genes also dictate how your body stores and distributes its fat. Exercise can only change the shape of your buttocks to a certain extent.

Diet and exercise

However, age and genetics are no reason to neglect diet and exercise.

A strong dedication to exercise can help build the muscle mass of your buttocks and give them a more appealing shape.

A healthy exercise routine can also help your glutes resist the changes that age and gravity can cause.

An early exercise routine can help maintain and lengthen the butt you had in your 20s and slow sagging and other hormone-related changes.

A quick history of butt contouring techniques

Buttock contouring, or butt lift, is a relatively new procedure in the world of cosmetic surgery.

Wounds, injuries, and deformities of the buttocks have been treated for a very long time, but the concept of shaping this area is still relatively new.

Its popularity has steadily increased as both the demand for body beautifying surgery and the safety of liposuction have increased.

The recent rise in popularity of the procedure can likely be explained by changes in fashion and the media – think of the influence that celebrities like J Lo and the Kardashians have had.

The first reported attempt to shape the buttocks took place in 1969. In this first attempt, breast implants were used to add shape to the patient’s buttocks.

The results were not very impressive. Over time, the procedure was performed more frequently, and problems with the movement of the implants became apparent.

These problems were quickly solved by placing the implants between the gluteus maximus and gluteus medius rather than in the subcutaneous layer.

As liposuction became safer and more popular, it became the most commonly used method for surgeons to remove unwanted fat from various areas of the body.

This eventually included the flanks, lower back, and areas around the buttock region. Liposuction was also quickly adopted as the primary means of shaping the buttocks.

If you are thinking about a buttock lift, there are two different techniques you can consider: the traditional buttock lift and the newer Brazilian buttock lift.

Both methods have their merits, and which procedure is best for you will depend on your individual body shape and goals.

The Traditional Butt lift

A traditional butt lift may include one or more of the following methods: Skin removal, Liposuction Butt implants.

Exactly what happens during a butt lift depends on the patient and her individual esthetic goals.

Skin is typically removed for sagging, liposuction may be performed to sculpt the buttocks, and implants may be used to add volume.

When implants are used, the procedure is called buttock augmentation.

Butt lift without implants

In this procedure, your surgeon will make a small incision in your buttocks to remove thin layers of excess fat, tissue, and skin.

Depending on your individual needs, the buttock muscles may be tightened. Sutures may be placed deep within the muscles and tissues to lift and hold your new butt shape in the desired position.

There are four main types of traditional butt lifts, all of which can use liposuction to shape the buttocks.

They differ in the type of incision your surgeon makes to correct your sagging: An upper buttock lift focuses on the contours of the upper part of your buttocks.

The incision is made across the curve of the upper portion of your buttocks, through which the underlying tissue can be removed with liposuction or repositioned to improve buttock cleavage.

If your problem is only mild to moderate sagging, your surgeon may remove the tissue from just the bottom of your buttocks.

This helps to reposition the fat in the lower buttock area and lifts the gluteal fold at the base of your buttocks.

A lateral incision can be made on either side of the buttocks to address mild sagging at the hips and outer edges of the buttocks. This method limits scarring.

A “butterfly” butt lift involves making an incision from the central crease (the “crack”) upward and outward. This technique is good for tightening sagging tissue, but it does not usually reposition or lift the buttocks.

Buttock lift and augmentation with implants

If you have a flat or poorly defined buttock – especially if you have very little subcutaneous fat – you may choose to have a buttock lift with implants.

In this procedure, the implants are inserted through an incision hidden in the central buttock crease.

However, depending on your needs, another incision may be better, and this can be done without any problems.

The implant is then placed under the gluteus maximus to give the buttocks a fuller, rounder appearance.

There are three main types of implants available:

Water-based implants, as you might imagine, are filled with a sterile saline solution. They are considered the safest option because, in the unlikely event of a rupture, your body will harmlessly absorb the water.

On the other hand, they are not popular because they can rupture if you bump them sharply or in the event of a hard impact, like a fall. They are also criticized for not feeling as natural as other options, as they tend to feel firmer.

Cohesive gel silicone implants are another option and are similar to breast implants.

Surgeons shy away from this type of implant, however, because of the amount of stress your buttocks are subjected to even during normal daily activities.

The likelihood of a gel silicone implant rupturing under the stress of your body weight increases.

Fixed silicone implants are the most popular type of buttock implants used today.

These implants, unlike breast implants, are completely firm yet pliable, with a feel and texture that fits your buttocks much better.

They come in a variety of shapes, or they can even be custom cut for you. And thanks to their solid design, they can’t tear.

The Brazilian Butt Lift

As liposuction techniques improved, a new butt lift technique finally emerged from South America: the Brazilian butt lift.

Invented by Dr. Ivo Pitanguy in Rio de Janeiro in 1992, this technique is actually called “autologous fat transfer” in the medical community.

Autologous fat transfer means that fat is taken from one part of your body to be put into another part of your body.

For a Brazilian butt lift, this means that fat is taken from a place where it is abundant and inserted into your buttocks.

It’s basically liposuction and butt augmentation, but instead of implants, your own fat is used for the extra volume.

The Brazilian butt lift is a two-step procedure: Using liposuction, your surgeon removes unwanted, excess fat from your body.

In addition to the buttocks, the most common target areas are the inner thighs, lower back, abdomen, and stubborn love handles.

Sometimes even fat can be taken from your upper arms if there is enough there. It is also often necessary to take fat from two or three different areas of your body to make sure there is enough to get the results you want.

In this first step, you get the benefits of liposuction: a slimmer and more toned body.

The fat that is removed is then medically treated and purified. This purified fat is strategically injected into your buttocks, reshaping them the way you want.

Unfortunately, not all of the transferred fat cells can survive. Current estimates by surgeons are that about 20 to 30% of the transferred fat cells die.

To counteract this, your surgeon may “overfill” your buttocks or perform a touch-up after a few weeks.

Ideal candidates

If you are interested in a buttock lift to improve the shape of your buttocks, you must first determine if you are a candidate for the procedure.

A good candidate for a traditional buttock lift should:

  • Be in good general and mental health.
  • Have realistic expectations.
  • Be a non-smoker or quit smoking. Cigarette smoke can have a negative impact on your ability to heal after surgery. If you are a smoker and still want a butt lift, you will need to stop smoking at least temporarily for a period of time before and after the procedure.
  • You have excess skin and fat in the buttock area that needs to be removed.
  • You have been working on getting rid of the excess fat and tightening the skin through proper diet and exercise.
  • You must also be willing to continue eating right and exercising to maintain the results.
  • You should not plan to lose any more weight as this may affect the results. Weight loss after surgery can have an unfavorable effect on your results.

Many of the criteria for ideal butt lift candidates also apply to Brazilian butt lift surgery, but there are a few additional factors.

Good Brazilian Butt Lift Candidates:

  • You want to add volume to their buttocks.
  • Have easily accessible pockets of excess fat that can be removed, cleaned, and transferred to the buttocks with liposuction. These pockets of fat are preferably located in the midsection or thighs. If you don’t have enough fat, you may need to get implants.
  • You will not have excess sagging skin.
  • Sagging buttocks are better treated with a traditional butt lift.
  • You have the right body shape for a Brazilian butt lift. Not everyone is ideal for this procedure, and your surgeon can help you decide if adding more volume to your buttocks will help improve their appearance.

This is because the nicotine in the bloodstream can have a negative impact on blood circulation and the overall healing process.

You will probably also be asked to abstain from alcohol for a few weeks.

Remember, by properly preparing for your butt lift procedure; you can have a smoother surgery and a faster recovery!

Recovery: Post-Operative Advice

After surgery, you will be taken to a recovery room, where you will be closely monitored for a while until the anesthesia wears off.

After a few hours, you will be allowed to leave the room, but before you go, you will be given a list of specific instructions to follow.

These instructions may include how to care for your buttocks and any areas treated with liposuction during your recovery, what medications to take, and what hygiene guidelines to follow.

Most likely, you will be told to always wear a support garment or compression garment after your procedure. Compression garments help relieve swelling and allow the skin in the suctioned areas to absorb better.

Traditional Butt Lift recovery

Whether or not you choose to include implants in your butt lift surgery, you will need to wear compression garments for at least 2 to 3 weeks. This will help the skin heal and form properly and give your new buttocks some much-needed support.

After this time, you will likely be able to gradually return to your normal activities, but it may take up to six months to see your final results after surgery.

It is only at this point that the swelling has reduced sufficiently, and the implants have settled into their final position.

Brazilian Butt Lift Recovery

If you choose to have a Brazilian butt lift, the recovery process will be slightly different. You will most likely be asked to avoid sitting for long periods of time or at all for at least two weeks (some surgeons prefer even longer).

This is important because sitting on your new butt for too long during recovery can damage the fat transferred there.

Excessive pressure prevents the formation of veins that supply the fat with blood and nutrients to help it survive.

You will be asked to sit with a pillow or pad under your hamstrings to help straighten your buttocks and prevent too much pressure on the fat grafts. You will also need to sleep on your stomach or side for the same reason.

During recovery, compression stockings are necessary to apply even pressure to the areas treated with liposuction.

Small drains may also be placed in the incisions under the skin to flush away excess blood or fluid. If your procedure includes these drains, you will be given detailed instructions on how to care for and clean the drains.

The drains are usually removed by your surgeon within a few days.

The length of recovery varies from patient to patient, but most people can return to work within a week or two.

If you have had a Brazilian butt lift, you will need to take frequent breaks from sitting.

Sometimes recovery can take longer, but most patients can expect to gradually return to their normal activities over the course of about 4 to 8 weeks.
